About Scabies in Humans

Scabies is a highly contagious, itchy skin condition caused by very small, wingless insects or Mites called the Human Itch Mite, mites on humas amd the Scabies Mite. While an adult Scabies Mite can range in size from 0.2mm to 0.4mm, which makes it just barely large enough to be seen by the human eye, it's nearly transparent body makes it almost impossible to see without the aid of a microscope. Since the scavies Mite burrows just under the skin, it is more likely that you will see the zigzag trail Scabies Mite and not the actual Scabie Mite itself. The scabies life cycle begins when the newly mated female selects a site on the skin for burrowing. The female Mite uses suckers on her legs (called ambulacral suckers) to attach herself to the skin surface and then cut a channel into the skin using her jaws (chelicerae), as well as the cutting edges on her first pair of legs. She burrows only in the top layer of epithelium (skin). Over her lifetime the burrow can reach 2-3 cm and is seen as the zigzag line. The female eats the skin and emits secretions as she is making her burrow. Once the female is established in the burrow she rarely, if ever, leaves it.
 The female scabies mite starts laying eggs within a few hours of burrowing into the skin. She continues to lay eggs at a rate of two to three times a day for up to two months. Larvae emerge from the eggs three to four days after they have been laid. The first stage of the larva is a very small six-legged Mite. After emerging from the egg, the larva moves out of the burrow and onto the skin where it searches for a suitable hair follicle to use as shelter. It lives in the hair follicle for two molts, approximately four to six days, before it becomes an adult. After completing all the molts, the Mite now has all eight of its legs. At this point the scabbies Mites return to the surface of the skin where the male Mites search for female Mites to mate with, starting the cycle all over again. The entire life cycle can be completed in 10-17 days.

Symptoms of the Scabies Rash
A very small, hard to see, zigzag blister usually marks the trail of the parasite as she lays her eggs. If the infection is left untreated, the amount of these trails may become so many that they begin to resemble a road map.

As anyone who's had scabies can attest, infestations are very uncomfortable, especially at night when itching becomes more intense. It's hard to exercise restraint when it comes to scratching the rash. However, excessive scratching can cause open skin sores on the skin that become infected with bacteria such as Staphylococcus aureus or beta-hemolytic streptococci, a condition known as impetigo. According to the Centers for Disease Control, this may even lead to a condition called post-streptococcal glomerulonephritis, or inflammation of the kidneys. Impetigo is treated with a prescription topical or oral antibiotic.
An extremely unpleasant complication of scabies is called crusted or Norwegian scabies, a skin condition characterized by crusty, scaly patches that cover much of the body in which many scabies mites are present---sometimes up to two million, according to the CDC. Particularly at risk are the elderly and those with autoimmune disorders or who are physically unable to scratch because of paralysis, injury to the spinal cord, or lack of physical sensation. Crusted or Norwegian scabies can be difficult to treat with traditional first-line topical treatments. Often an oral medication called ivermectin is prescribed.
Scabies can be adequately and promptly treated, as long as a patient's condition is diagnosed by a physician, treated using a first-line medication and necessary steps are taken to purge the patient's home environment and personal belongings of the scabies mite. However, if a patient fails to use medications known to treat scabies, uses them incorrectly or repeatedly exposes themselves to an environment where scabies are present, attempts to get rid of an infestation can fail.
This skin condition is highly contagious; all it takes is one female scabies mite to cause a human infestation. Children can give scabies to playmates, and adults can transfer the mite to sexual partners. Informing everyone with whom you've had close physical contact is necessary to keep scabies from spreading.
To avoid complications of scabies, see your doctor to make sure that your rash is caused by an infestation of this particular mite. Your doctor will most likely prescribe a topical medication that contains either permethrin or crotamiton that is applied to the body from the neck down and left on between eight and 14 hours, after which it can be thoroughly rinsed off. Residual itching after treatment is common; although the mites are dead, their proteins remain under the skin, causing an allergic reaction for up to two weeks. Taking cool baths, using a soothing lotion such as Calamine or taking an oral antihistamine (if recommended by your doctor) can reduce post-treatment symptoms.
A crucial step to avoid further complications of scabies is to make sure that all of the clothing and undergarments you wore two to three days before treatment is thoroughly washed in hot water and dried on high heat. Bedding, linens, towels and cloth items should also be laundered as well. If you can't wash it, store it in a sealed container and place it outside of your home. (The American Social Health Organization recommends storage for at least two weeks, while the CDC advises three days.) Vacuum carpeting, rugs and furniture well and dispose of the bag outside of your home immediately.
